HR jump 3k into 5k run

Hi, I'm pretty new at all this but have become an avid ParkRunner seeking sub 20min. Giving it all I'm sub 21 now and cannot see where to gain. Any advice?

I'm in Singapore so it's v.hot and humid.
I try and do either longer slower runs or speed intervals during the week. Eat, hydrate and rest beforehand.
1st 3k are OK. Then I crumble at 4k- so hard to keep pace and breathing is wheezy (asthma meds don't help). Looking at my HR there's a gradual increase then marked step up about 10-15bpm @ 3k then another 5bpm at 4k to my max. It's a long tortuous last kilometer!
