Help needed to interpret fitness data

Had a fitness test on an exercise bike as part of a medical today. Am 35yrs old. My HR was made to go up to about 155bpm, but they had to load up the effort to 250 (can't remember the units watts-hour???) to cause the HR to rise which it finally did. Was told 155ish is about 85% of my estimated max HR. Therefore max HR is about 185ish bpm. VO2Max was 53.8 and RHR was 49bpm.

Other metrics were taken (heart efficiency, breathing response, lung efficiency) which were all OK.

Last one was about metabolism, respiratory exchange ratio, RER.
RER from 0.7 to 0.85 shows fat used.
RER from 0.85 to 1.0 show carbs being used.
Over 1.0 is aneaerobic.
RER went over 0.85 at HR 120.
RER went over 1.0 at HR 135.

Is RER 1.0 my lactate threshold?
I expected my HRMax to be a lot higher. Is it low because of the high VO2Max?
