Has to be the place to start, take the twisted links out and see if it fixes it, even if the chain is then too short so long as you avoid the big ring/big sprocket combo you can do a test ride to see if that was the problem. If it really is twisted then that is likely to causeproblems so no point speculating about other causes til that is eliminated.
Thinking about it - if its worn - its teeth aren't deep enough to keep a firm grip of the chain - hence why its falling off both outside and inside - its getting pulled off by the different chain lines ?

thinking about it, talking endlessly about what you are going to do and obsessing about the details ( including chainrings) and the plan is a great way of avoiding actually doing it!
I picked up a new bike, a Focus Cayo, a few weeks back and as much as I like the bike it has one very irritating issue. The internally routed rear brake cable bangs against the inside of the frames top tube on anything but a perfectly smooth road surface.
I've taken it back to the shop where I purchased it from twice but both times they've failed to fix the issue.
A quick Goggle search shows this is not an uncommon issue on certain frames but I'm not convinced by the various fixes people used (uncut zip ties around the cable inside the frame, shoving bits of foam insulation inside the frame) so decided to take a look myself last night.
With the forks out of the frame I was able to 1) retrieve x2 cable grommets the bike shop had lost inside the frame and 2) observe there is no guide tube for the brake outer cable in the top tube. The outer cable basically runs as one long length from the brake lever to the calliper, with part of the cable hidden inside the frame. The cause of the rattling is due to the cable grommets not sufficiently securing the outer enough to keep it taught inside the frame.
My fix was to place some o-rings (like these - http://www.halfords.com/cycling/parts/brakes/halfords-cable-brake-o-rings) over the section of outer cable that feeds inside the frame and place small zip ties on the outer at the points where it enters and exits the frame. With those two zip ties I’m able to push them against each other and get greater tension on the outer cable running inside the frame. I only had x3 o-rings available last night so might pick up a more today, foam ones if possible, so there are more rubber/foam to frame contact points.
A quick spin last night seems to have fixed the issue but a longer test ride is needed.
Has anyone else suffered with this issue and if so how did you fix it?
Has anyone used the zip ties inside the frame method, did that work?
replaced chainset with ultra 6700. moved the front mech to accommodate . problems solved.
will retry the original with adjusted mech - just to make sure it was the chainring and not the mech location.
not so much clueless, but checking dave. never come across a chain ring that actually turned to crap before. wear and tear wasn't an issue on the old campag 8spd that ever impacted riding the damned thing.
no need for a 65minute drive to Royles for a numpty issue. on a bike I don't have ( should the OH ask at this juncture )

Mine is the 2015 Cayo 7 model, not sure if the cable routing is the same. According to the bike shop they've had a few owners complaining of rattling cables
Don't forget to let the CO2 out of the tube when you get home and then pump up with your trackpump. The CO2 will leak out quicker than air and you'll end up with a flat.
You could always go and hang around oversize baggage at an airport. There will be loads of cyclists ditching CO2 canisters when they realise most airlines won't carry them!

sounds like a bent link if it has a twist in it - I have never seen one though - which is kicking the chain off.

obvious question : i dont have to replace 10spd ultegra chainset with a 10spd ultegra chainset.

i think it does have to be shimano hollowtech or its a new bb as well. ( BB is ultegra 24mm? )

I had my first puncture in years yesterday and used my shiny new CO2 cartridge.
This was great but what I want to know is " will that one use have emptied it or am I likely to inflate another tyre with it?
I will get a new one shortly but maybe not in time for my next planned ride tomorrow.
